Sorry for the false alarm! It was indeed a plugin that was causing the problem even though com_kunena was in the exclusion list.
The plugin causing the problem is NSTS - Non-SEF to SEF Version: 3.1.0 - 2015-08-01 which has an option to exclude components, unfortunately it doesn't seem to be excluding kunena in my case.
Hopefully the developer will fix the problem in a coming release.
Thanks for the input and sorry to have wasted your time.

I was testing posting as a guest and after logging in with an admin account the preview doesn't work for logged in users. I have tested using both kunena templates and it's the same in both. Firebug isn't showing any error messages in the console.
I disabled all the plugin's assigned to members of the special group and that made no difference.
Any thoughts?
As an aside:
When using the Crypsis template guests can access the quick reply function which circumvents the captcha feature which would cause a bit of a spam issue on a live site.
The Blue Eagle template was throwing capture error when posting as guest, even though all extensions including kunena's security (captcha) options were disabled and all input fields nulled. I finally gave in and added reCaptcha keys to overcome the problem. The Crypsis template worked fine with the same extension settings i.e. no reCaptcha keys entered.

This was discussed a while ago and I am not sure what was the outcome. The previous discussion is >>> here <<<.Celt wrote: When using the Crypsis template guests can access the quick reply function which circumvents the captcha feature which would cause a bit of a spam issue on a live site.
Setting up CAPTCHA in K 4.0 is complicated. There are Joomla's CAPTCHA plugin settings and there are Kunena configuration settings for CAPTCHA. The Joomla reCAPTCHA plugin settings work with the Kunena Crypsis template. The Blue Eagle template does not use the Joomla reCAPTCHA plugin settings; the Blue Eagle template uses the Kunena CAPTCHA settings. We have had this discussion before: see Captcha not working and Recaptcha 2.0? .Celt wrote: The Blue Eagle template was throwing CAPTCHA error when posting as guest, even though all extensions including kunena's security (captcha) options were disabled and all input fields nulled.
I don't blame you if you are confused. CAPTCHA and Kunena is a confusing topic. However, as you say, I don't think the CAPTCHA settings have anything to do with why your message preview is not working.
If you want to talk more about Kunena and CAPTCHA, I suggest you use another topic for that discussion.

Thanks for the info and links. I didn't really want to add reCaptcha to the site for numerous reasons. But I couldn't switch templates for testing without it. :S
I had sussed that the two templates were handling the captcha differently and they both seem to work now that the keys have been entered in all the relevant places.
As you say Captcha probably isn't the cause of my ongoing issue with the preview not working. Captcha should be off for trusted users and they are the ones who the preview isn't working for. Guest using captcha have no problem with the preview either nor do logged in/registered users.
I disabled the Captcha entirely and the behaviour remains the same which rules it out.
There seems to be a Captcha issue with the Blue Eagle template which behaves differently to Crypsis. Blue Eagle seems to ignore the the Display Captcha for Nobody directive and the only way to disable it is to remove the keys. If you remove the keys and set it to Nobody Kunena frontend throws a Captcha config error for guest posting. Crypsis seems to work as expected based of the rules in the Kunena config.
Should I open a new thread for the Admin preview issue as, it's not really the same issue as I had when I opened this thread?
