The next version of Kunena will have many sql optimizations for just this problem. You are not alone my friend. The queries that are required for Fireboard and Kunena need a lot of work. Much of this has already been done with much success. It is already running on one of our dev's sites and has shown improvement of speed in the optimized sql queries he is running. I believe he has said that his sql load has dropped to between 5-20 percent, which is a dramatic improvement. We will have to wait on the next version for the optimized queries but rest assured we are working hard to get there.

Wow, do you really have over 19 million messages in your site!?!
No wonder that Kunena is slow; current version barely manages to handle 200 000 messages without serious degrade in performance! Your database server is not slow, it just is working hard to keep up with huge requests.
Do you have SimpleBoard and Kunena running in the site at the same time? It will make things worse in huge forums like that, because of MySQL has two huge tables to be read instead of one.
Can you tell more from your environment?

Can it be that some of the indexes are missing?
If you have upgraded, the Kunena installer will not add missing indexes that should have been in a prior release.
Can you check whether jos_fb_messages
has indexes on:
thread
id
hold
?
Thx!

Rows_examined: 19440020 <- there must really be something wrong in your database!
As fxstein said, you seem to have no indexes in your database and every time you query database, the whole table will be read (many times). Indexes are added during update, so it needs some manual work.
