Regarding Captcha, you will have to do something.
My web hosting provider is threatening to shut down my forum if better security is not provided.
It is full of fake mail that my provider doesn't like and they have shut down my mail that notifies users of new posts etc.
This is all I have to choose from and nothing works or is good enough

Registration is done by joomla. In Joomla 5 you can install this
plugin
and configure it with the right keys.
As far as I can see you forum is not accessible to the public so I wonder where the huge amount of emails is coming from.
A good hoster normally allows at least 500 - 1000 emails per day.

But this has nothing to do with subscription emails, as spammers don't usually subscribe to categories or topics, unless you use the forced subscription plugin Autosubscription for Kunena.
A Chaptcha can only help to the extent that fewer spambots register, but it does not prevent real spam users from registering. For Kunena herself, a captcha only makes sense if you grant the public the right to write.
Spam users or spambots can post spam, but cannot send mails from Kunena. So it can only be subscription mails from categories or topics that your users have subscribed to.
You can prevent too many mails from being sent at the same time by setting the value for E-mail to Multiple Recipients in the configuration under Subscriptions to a value higher than 0.
However, a valid e-mail address must then also be entered in the Visible E-mail Recipient field. An email is then sent to multiple recipients. The higher the value, the fewer emails need to be sent.
Important! Always create a backup before you make any changes to your website!
I don't understand the connection. If a spammer or spambot registers, it must have a valid email, at least for a short time, otherwise the registration cannot be completed. It is full of fake mail that my provider doesn't like and they have shut down my mail that notifies users of new posts etc.
